Is all salary subject to Medicare taxes?

I am looking at payroll information for a government employee. Gross salary of $138,032.45 and Medicare taxes withheld of $1,993.42, Of that gross salary $555.46 is categorized as 'Exempt Wages'. What could be exempt?
