how to report taxes if employer didnt withhold

What is the best way to prepare taxes for this client? he worked at a restaurant as a waiter, and got paid cash. He wants to do his taxes, there are basically 3 questions
should I do his taxes as if he had been self employed? what happens in case of an audit
or should I use tax form 4852 Substitute for w2 wage and tax statement.? if so will this get the employer in trouble or make the employer pay the fica taxes they didn't withhold?
