Hi!
I have been all over the web searching for such an animal but every form I download has a green and shaded background like the preprinted one the IRS gives out. For some reason the color bothers me and doesn't print right on my one-color Laser anyway. I have in my possession a printed form like this but I can't find a blank one. All the links I have followed always led to the green one. Since the form I have came from an accountant I was hoping that someone here might know of a source.
